Application For Scholarships To The Students Studying In Oriental Colleges
** Name of the student in full and Sex
** Name of the Institution with full address quoting Taluk and District
** Whether Institution is recognized
** Name of the Father or Guardian
** Occupation of the Parent/ Guardian
** Income of the Parent/Guardian from all sources exclusive of allowances [allowances should be shows separately
** Whether the student is in receipt of any other scholarship in the previous year. If so whether it is renewable [Name of the scholarship and amount of scholarship should be furnished]
** Whether the candidate belongs to Scheduled Caste/Scheduled Tribes SC/ST
** Whether the candidate is Physically Handicapped, if so the nature of deformity
** Class and the course in which studying [whether it is Sanskrit, Telugu, Urdu, Arabic, Hindi or Persian]
** Marks obtained in the previous public examination conducted by University or Commissioner for Government Examination in each subject]
** Aggregate percentage of Marks Signature of the Candidate : Signature of Father/Guardian
** Remarks of the Head of the Institution [Progress and Conduct]
** Complete Postal address of the Institution
