Medium : English/ Gujarati

Instructions :
1. Incomplete form will be rejected. It is compulsory to attach a self attested photocopy of mark sheet of class V, VI, VII, VIII, IX, X & XI as applicable and 4 recent passport size photographs to the Pariksha form.

2. It is compulsory to provide mobile no. & e-mail id of the student / parent / guardian. The result of tests or any other information by the RJ VISION PVT. LTD. will be sent to the student via SMS and / on E-mail ID only.

3. During the YCCP (Yearlong Classroom Contact Program) & PFC (Pre Foundation Course) at RJ Vision Pvt. Ltd., a student must meet the following requirements failing which his/her scholarship program will be cancel with immediate effect. The decision of Managing Director (RJVPL) shall be final and binding to the students and the parents.

(i) The attendance of the student in the classes should not drop below 80% throughout the classroom contact program. It is compulsory for a student under this program to write all his/her Test Papers conducted by the Coaching institute in their batches.

(ii) The students will be required to maintain a minimum of 70% marks in their regular assessment tests and all the tests conducted by coaching institute (RJVPL).

(iii) Any act of indiscipline by the student brought to the notice of the administration of RJVPL in the institute, will not be accepted. A warning will be issued to such students, offering them a chance to improve their behavior. Despite warning, if the student does not change his/her attitude and commits another act of indiscipline further, then his/her scholarship program and admission will be canceled with immediate effect.

(f) If a student remains absent for more than two weeks due to any circumstances without prior permission from the Institute, then his/her scholarship program will becanceled.

4. To be eligible for cash awards, it is mandatory for the students for each class i.e. Class VI, VII, VIII, IX, X, XI & XII to join our DLP Course before 31st JAN, current year, irrespective of whether they join our Classroom course or not. Once the DLP enrollment is done, it will not be cancelled in any case. The students will fill separate enrollment form for DLP. No fees will be charged for DLP Course. It will be 100% Free.

5. If any student fails in his/her school exam, this ‘PARIKSHA’ facility will be withdrawn & the student will be disqualified from the Scholarship program.
