Conditions:
1. The application is to be filled in the candidate’s own handwriting
2. All entries must be substantiated by enclosing original certificates, however, marks proforma need not be submitted.
3. All entries must be complete and unambiguous. Incomplete and ambiguous entries will be summarily rejected.
4. Enclose list of original certificates submitted by you.
